Turkey: Special Needs Mental Rehabilitation Training

Background

Relief International (RI) is a humanitarian, non-profit, non-sectarian agency that provides emergency relief, rehabilitation, and development services throughout the world. RI operates in 19 countries across Asia, Africa, and the Middle East and has extensive past performance responding to emergencies. RI’s goal is to empower the most vulnerable communities to develop lasting solutions to their most pressing challenges and to build an environment that creates permanent well-being.

In Turkey, the objective of RI’s programming is to fill the gap in health service delivery for refugees, particularly niche services that are not envisioned to be part of the migrant health centers. RI currently supports six mental health centers and three comprehensive physical rehabilitation centers across Turkey through five local partners with funding from the European Civil Protection and Humanitarian Aid Operations (ECHO) and the US Bureau of Population, Refugees, and Migration (BPRM). At the request of the Turkish Ministry of Health (MoH), RI is also undertaking dental hygiene promotion activities and a leishmaniosis prevention campaign with implementing partners targeting refugee communities.

Objective

Relief International (RI) plans to contract a training institution that will arrange and conduct a training for the mental health projects implementing partners’ Special Needs Rehabilitation (SNR) specialists in ARABIC aiming to: provide three basic components for the preparation of the SNR unit in the mental health centers (MHCs) on diagnosis, assessment, and intervention and building therapeutic plans. The contracted supplier is expected to organize the whole training delivering the technical knowledge, the technical tools, and the logistic arrangements.

Training technical framework

The training must covers the following main components with the relevant details included:

1.Diagnosis:

The aim of this component is to train SNR Specialists and staff to identify the distinctive symptoms of each category of disorders in children in addition to the developmental disorders according to the fifth edition of the Diagnostic Statistics Manual (DSM5) of Mental Disorders.

The required types of disorders covers the main categories mentioned in DSM5 including Childhood Disorders and Neurodevelopmental Disorders.

This component must include a review of the treatment trends of each type of neurodevelopmental disorders.

The training must cover the basic concepts on the prevalence of each disorder and its contributing factors, as well as the indicators of severity and frequency

The training must addresses the following disorders:

• Global Developmental Delay

• Adjustment disorders,

• Reactive Attachment Disorder,

• Disinhibited Social Engagement Disorder

● Disruptive, Impulse-control and Conduct Disorders

• Oppositional Defiant Disorder,

• Intermittent explosive disorder,

• Conduct disorder,

• Pyromania,

• Kleptomania

● Communication Disorders

• Language Disorder

• Speech Sound Disorder

• Childhood Onset Fluency Disorder

• Social (pragmatic) Communication Disorder

● Motor disorders

• Developmental coordination disorder,

• Stereotypic movement disorder,

• Tic disorders (Tourette’s disorder, Persistent motor or vocal tic disorder, Provisional tic disorder)

2.Assessment:

This component is on assessment of SNR cases through the use of specialized tools for measurement, and mental and behavioral assessment as well, in addition to a detailed profile of the indicators of mental, emotional, social and physical performance.

This component must focus on the use of “Stanford Bennett 5 Test” which includes (liquid intelligence, knowledge, quantitative inference, visual processing, and working memory). These fields summarize the basic mental functions of learning and adapting to the physical and social environment.

The training on this component will cover the measures of the adaptive behavior according to DSMs, which covers the areas (social, practical, conceptual and motor). There will be (16) sub-tests related to the previous area in addition to the scale of autism spectrum disorders according to DSM5, and the scale of ADHD according to DSM5.

3.Intervention & Building Therapeutic Plans:

This component addresses the skills of identifying the objectives and intervention tools through indicators of diagnostic, evaluation and building an individual plan based on an in-depth study of social, cognitive, emotional and physical variables.

And the use of methods of functional analysis of behavior and principles of behavioral modification and examples of programs and global plans dedicated to intervene with children with of neurodevelopmental disorders in order to reach an individual intervention plan using the following steps:

  • Identifying the intervention team

  • Identification of the targeted behavior / to be trained on

  • Collection of information

  • Collect information on the targeted behaviors

  • Conduct additional assessments

  • Sample of functional evaluation note

  • List of repetitive problematic behavior

  • Building the intervention assumptions

  • Building an individual intervention plan

  • Define the behavioral and procedural objectives of a multi-specialized team

  • Behavioral planning of class situations

  • Involve parents with intervention plans

  • Conduct monitoring and evaluation

SNR Technical Tools:

The supplier must train the participants on all the SNR related technical tools.

Additionally, the supplier must provide RI with all of these tools that will be; based on the contract, a property of RI.

RI has the right to share, print, modify, use these tools as per this agreement and the supplier doesn’t have the right to claim the ownership of these tools later on.

However, RI is obliged to acknowledge the supplier’s intellectual rights through referencing these tools.

Logistical arrangements:

The supplier is expected to deliver the requested deliverables within the timeframes mentioned below as well as the related logistical arrangements that mainly includes the trainer, facilitator, training curriculum, per and post-tests, accommodation for participants, visas, transportation and accommodation for trainer/facilitator, catering arrangements...etc.

These arrangements are mentioned below in details.

Training timeframe

1.First component: Diagnosis:

The supplier is expected to cover the first component through at least 21 twenty one training hours over 3 three training days.

The supplier is expected to deliver this component during 4, 5, 6 March 2019.

2.Second component: Assessment:

The supplier is expected to cover the second component through at least 35 thirty five training hours over 5 five training days.

The supplier is expected to deliver this component during 7, 8, 9, 11, 12 March 2019 considering:

Sunday 10 Marchto be a training break day.

RI recommends the supplier to use this day to conduct a staff-care activity for the trainees aiming to enhance the team spirit as well as to support them psychosocial wise based on direct consultations and guidance with RI MHPSS team.

3.Third component: Intervention & Building Therapeutic Plans:

The supplier is expected to cover the third component through at least 21 twenty one training hours over 3 three training days.

The supplier is expected to deliver this component during 13, 14, 15 March.

Requirements:

The supplier is expected to deliver the requested technical training and tools through the below settings that are expected to be fulfilled by the supplier:

I. Location: the training location should be in Gaziantep.

II. Accommodation: Accommodation is required in a suitable hotel for the whole training period including the day off. Hotel’s location is recommended to be easily reachable in a safe and secured neighborhood.

Trainees are expected to:

Check-in on 3 March afternoon

Check-out on 15 March noon time.

12 nights

RI will not cover any related to no-show, early check-in, late check-out, absence, minibar usage cases cost or any irrelevant cost. It is the supplier’s responsibility to communicate the message to the trainees and cover any related costs, if incurred.

III. Training venue: A suitable venue that fits at least 20 participants comfortably is required with good ventilation system (a basement venue is not preferable).

Water bottles, coffee, and tea must be available and served at the training venue for all participants all training time.

It is highly recommended that the training is held in the hotel where participants are accommodating.

IV. Catering:

  1. Breakfast: an open buffet breakfast with a variety of high quality fresh food is required on a daily basis for all the participants;

  2. Coffee breaks: two coffee breaks (coffee, tea, cookies, etc.) are required each day, one in the morning and the other in the afternoon. Likewise, coffee, tea, and water must be available for the participants throughout the workshop.

  3. Lunch: a full board lunch meals including a variety of fresh food with the option of meat, chicken, fish, and vegetarian ones, in addition to appetizers, sweets, and fruits.

V. Transportation: if the training venue and the accommodation hotel are in separate locations/areas, the supplier must provide and ensure the safe and adequate transportation of the trainees between the two locations.

VI. Communication: RI is responsible of providing the list of trainees with their contact info, as well as connecting the supplier with a focal point from RI’s implementing partner in order to facilitate the training arrangements. The supplier may have direct communication with this focal point as well as the trainees, still, however still, no decision can be taken by the supplier without RI verification.

VII. Visibility: through direct guidance from RI, the supplier must adhere to ECHO/RI visibility standards.

VIII. Other arrangements: the supplier is responsible of handling all other necessary requirements (such as visas, permission from authorities, etc.)When applicable, RI can provide support.
